  • Physical Therapy is a treatment option for all ages that focuses on improving movement of the body and promoting a pain-free, active lifestyle. Many use PT as an alternative to surgery or medication, others just want to improve their health and prevent future injury. Whatever your reason, Ascent Physical Therapy can complete your goals by implementing a custom tailored plan for you. This could include hands-on manual therapy, exercise programs, and one-on-one patient education with a Physical Therapist.

  • Ascent believes that Physical Therapy is a great option when considering pain management or mobility improvement options. If you have specific areas of concern, you can always give us a call and we can have a therapist chat with you about your options and schedule a complimentary consultation to see if Physical therapy would be a good option for you.

  • Your appointment can be scheduled over the phone or in person when you have your prescription from a physician. After your evaluation, we will work with your schedule to book any future appointments. If you are interested in making an appointment please give us a call or fill out a form here and we will call you to schedule. We encourage you to book ahead because appointments do tend to fill up quickly.

    We are located on the north-east corner of the intersection of Square Lake and Telegraph road in a big brown building next to T-Mobile. If you are coming down Telegraph from the south, we are the first right turn after you cross Square Lake Road. If you pass Jimmy John’s or the Golling Dealership you have gone too far.   • On your first initial appointment you should bring your ID, insurance card, and your prescription (if you have a physical copy). We also advise that you dress comfortably; loose-fitting clothing and closed-toed shoes are recommended.

  • On your first visit we would advise you to come in 10-15 minutes early to help with the registration process and allow enough time for you to fill out paperwork. After paperwork is completed, we will bring you to one of our offices to begin the evaluation. This typically begins with collecting information on your background and medical history with the Therapist, as well as going over your personal goals for Physical Therapy. After this introduction, treatment can begin! At Ascent, we are known for our hands-on orthopedic approach, which we begin right on day one for approximately 15 to 30 minutes. After this, you may receive a modality to finish out your session. When your treatment session has been completed, we will work together with you to ensure you are scheduled for further appointments so we can best meet your physical therapy needs.

  • Yes, your prescription can be used anywhere you would like, even if it has the information for another clinic on it. The only requirement for a prescription is that it has to have been written within the past 90 days for it to be valid. In addition, if the physician noted a specific duration on the prescription, we cannot use the prescription if that duration has passed.

  • Each individual patient that we see is different, we do not have a set formula for how often someone needs to be seen. Once your Physical Therapist has evaluated you, they can recommend how often you should be coming in. The prescription from your Physician may specify a certain number of visits, but we will make sure that you will be seen as long as it takes to reach your Physical Therapy goals.

  • Yes, we do offer free 15 minute consultations. During this session, you will be able to meet with a therapist and discuss your concerns, but there will be no hands-on work done until your initial evaluation appointment. Typically, the consultation is for people who have not already received a prescription for Physical Therapy from their Doctor.

  • All insurance companies offer different amounts of coverage, but we can give you an estimate of your expected responsibility when you call with your specific insurance information. Typically, all that we need to find this information would be your enrollee ID/member ID and your date of birth. Of course, we can only estimate your expected responsibility - the final amount may differ once we bill your insurance directly. We would ultimately recommend that you call your insurance (using the number on the back of your card) and ask them exactly what you are expected to pay for Physical Therapy services.

  • Once your Physical Therapist has evaluated you, they can recommend how often you should be coming in. Your first appointment is typically an hour long, and each following session is about 45 minutes. If you are ever in a time-crunch, you can always let us know at the start of your visit and we will be sure to get you out of the clinic on time.

  • At Ascent Physical Therapy, we value each client's time equally. We understand that, on occasion, unforeseen circumstances make it impossible to attend your scheduled appointment. If this happens, please give us as much notice as possible so that we can reschedule your appointment and open that time-slot for another patient. We require that you call by 5:00 pm the previous business day to cancel your appointment.

    The first same-day cancellation will be waived, however, subsequent same-day cancellations will result in a charge of $50.00 per incident. This charge is the responsibility of the patient, and will not be covered by insurance. If a patient cancels without reason or “no-shows” more than two times, then we reserve the right to inform the referring physician and discontinue treatment.
